    Being a swap, Vacuum seems likely...when you get a vacuum leak, air is coming in unmetered, which leans things out. This will oftentimes create a high idle situtation. This may lead the IAC or TPS to overly compenstate and restrict air and fuel more...the subsequent increased resistance to putting it in gear leads to it not being able to correctly compensate the throttle settings and it dies. Why it doesn't restart, less sure...but probably due to trying to keep a normal idle with a vacuum leak has left it in a poor posisiton to start.

    Old vacuum lines will generally tear at the edges when pulled off as what happened pulling the eginge. Putting them on again over the lip certainly didn't help either.

    The haynes should have an alright trouble shooting guide for this car though...go through it and see what it says about stalling in gear, trouble restarting.
